If you’re living with a chronic medical condition, doctors at the location of ANMH and the wider team of allied health practitioners can work with you to create a personalized care plan that supports your ongoing health needs.
What is a chronic condition?
Chronic conditions are long-term health issues that have lasted six months or more. Some common examples include:
- Asthma
- Cancer
- Cardiovascular disease
- Diabetes
- Musculoskeletal conditions
- Stroke
What is a GP management plan (GPMP)?
A GPMP is a structured care plan developed in partnership with your GP. It outlines your specific health needs, the services your GP will provide, and steps you can take to better manage your condition. This plan helps ensure your care is coordinated and tailored to your goals.
Team care arrangements (TCA)
If your condition requires input from multiple health professionals, your GP may also put in place a Team Care Arrangement. This involves your GP working closely with at least two other healthcare providers—such as physiotherapists, dietitians or specialists—to ensure you receive the right support across all areas of your care. Having a TCA from your GP often results in heavily subsidised or bulk-billed sessions with your allied health practitioners.
Is there a cost?
Eligible patients can access care plans at little or no cost. To find out if you qualify, simply book an appointment with your GP. We’re here to support your journey to better health, every step of the way.
Your care plan will be reviewed regularly by your GP to make sure it stays aligned with your needs. This gives you and your doctor the chance to review your progress and update your goals as needed. Our practice nurse is also available to help monitor your condition and support you between appointments.
